The passenger simply sits onto the pad and fastens the seatbelt as normal, making sure the 'lap' part of the bell sits just under the bump. Then, the passenger moves the two elasticated flaps over 'lap' section, which ensures it stays there for the journey and doesn't creep up over the abdomen.
